Our connected health solution, Integrated Care Platform (ICP), supports people with long-term conditions to monitor their own vital signs from home via a myClinic Connected Health hub, with the reassurance that if readings go beyond set limits, their care team will be informed. 

Our patient management system ICP triagemanager enables the patients’ care team to closely monitor their condition and intervene before a crisis. 

ICP also offers video conferencing, allowing patients to maintain face-to-face contact with their care team regardless of location or medical state.

Tunstall's ICP connected health solution helps to support people with long-term conditions by enabling them to monitor their vital signs and symptoms daily in their own homes, with the reassurance that if their readings go beyond the limits set for them, a clinician will be informed. Connected health improves the patient's understanding of their condition, which in turn increases their confidence and empowers them to manage their own health with the support of their health and care team.

How connected health works

Vital signs

Tunstall's connected health solution is tailored to the needs of the individual patient, and a number of medical grade peripherals are available to support and manage long-term conditions. 

The data collected via the myClinic Connected Health hub is securely transmitted to the triage monitoring centre for review by a clinician.  Abnormal readings for the patient are flagged for follow up and intervention such as clinical coaching, nursing visit or GP appointment. The collected data may also be passed on to the patient’s doctor for further review or intervention.

The ICP solution allows the patients’ care team including community nurses, GPs and specialists to closely monitor the patient’s condition and intervene before it reaches crisis level and results in hospitalisation. ICP also offers video conferencing at the press of a button, allowing patients to maintain face to face contact with their clinician regardless of their location or medical state.

ICP triagemanager

Tunstall's patient management system ICP triagemanager provides clinicians with ongoing access to patient health information and trend data. ICP triagemanager can integrate with other patient management systems and enhances clinician workflow and standardisation, allowing patients to be prioritised on a needs basis.

ICP video conferencing

The ICP video conferencing capability is available with Tunstall myClinic Connected Health hubs. The video conferencing module provides a visual and audio link which allows the patient or their onsite clinician to interact or consult with an offsite clinician in a separate location. High definition, multi-party video conferencing and scheduling capabilities are also available.

ICP patient and carer portals

Tunstall’s patient and carer online portals are designed to accompany our ICP connected health system to provide mobile reporting. The following optional modules are available with Tunstall’s ICP connected health solution:

  • myCare portal: provides patients and carers with a real-time dashboard of their health information, including key vital sign records and health results. Accessible from any internet enabled device via the secure web portal, providing feedback to both the patient and carer on their progress.
  • myServices portal: enabling clinicians and carers to coordinate secure video conferencing and concierge services for their patients.
  • ICP Reporting portal: a secure web platform for clinicians and care providers. Developed to assist clinicians with providing secure reports on patient vital sign data and clinical alerts, triage notes, GP visits, video conference appointments, and more.
