Tunstall’s complete connected health solution, Integrated Care Platform (ICP), supports people with long-term conditions to monitor their own vital signs from home via a myClinic or mytelemedic Connected Health hub, or the new myMobile app. The multi-user myClinic hub is also available for community settings, enabling patients to use the same connected health equipment in a communal location. Patients can feel reassured that if their readings go beyond set limits, their care team will be informed.

The myClinic Connected Health hub is a component of Tunstall’s ICP. It’s a tablet device which uses wireless technology enabling patients to monitor their own condition from home. The myClinic health interview guides the patient through a series of measurements and asks pre-set health questions, alerting the patient when their scheduled interview is due. Patients use a series of medical-grade peripheral devices (such as thermometer, pulse oximeter, weight scale) to measure specific vital signs and health questions are set to the individual.  

The patient’s readings and answers are then automatically transmitted to Tunstall’s ICP triagemanager patient management software for review and processing by one of Tunstall’s Registered Nurses (RNs) or the patient’s care team.  Tunstall’s RNs check-in regularly with patients either via phone or video conference. Any deviations outside of the pre-set health parameters alert our RNs to further investigate and contact the patient, their GP, and care team.
